Patient choice and individualized care are essential for optimal management of type 1 diabetes. Levemir is the only basal insulin analogue that has randomized controlled trial data supporting its safety and efficacy in pregnant women with type 1 diabetes, with this distinction recognized by the ADA Standards of Care. While observational data exists with other basal insulin analogues, there are additional challenges that may exist (such as the prolonged duration of action of insulin degludec with long half-life leading to inadequate ability to rapidly titrate insulin to meet the demands of changes experiences during pregnancy) that is explicitly described in the ADA Standards of Care. I support the movement to prioritize and maintain patient choice, especially when it comes to the insulin choice for optimal management of type 1 diabetes.
