1. OOS of finished product might have occur due to failure of proper storage of API in the Warehouse which is subsequently used for that product but it is not covered in the guideline in which Phase(Phase I or II) it should be taken into account for investigation. 2. Is it really necessary to raise formal deviation investigation at the start of OOS Phase II investigation?
