PROPIN CONFIDENTIAL DOD-2026-OS-0958 OMB 0704-CMQE FR DOC. 2026-13631 Title: Certificate of Medical Qualification Examination Department of War, Office of the Secretary of War for Personnel and Readiness I am Requesting Approval of the agency collection and for the Exemption for the Application, under the Department of War, the Secretary of the Department of State, the Administrator of the Environmental Protection Agency, the Department of Justice and Attorneys General, the Secretary of Homeland Security, the Department of Transportation, the Secretary of the Department of Interior, the Directors of the Office of Management and Budget for their Approval, the Presidential Determination 2026-02 dated November 21, 2025 Memorandum for the Secretary of State and President Trump's Determination of the Memorandum of Justification, under the Trafficking Victims Protection Act and the Act to Venezuela would promote the purpose of the Act or is otherwise in the National interest of the United States, on my behalf, as Victims of the Act, as Civilian Personnel of the Federal Government, and under the Administrative Procedure Act as amended, by my Oath of my affirmation by reference of incorporation by association and import, for my Approval, Authorization, voluntary Consent, for the Enforcement of the Order to maintain and enhance health and readiness. Effective 07/06/2026 for immediate effect upon review. The Certificate of Medical Qualification Examination is providing the functional and environmental requirements of the duties of the position of employment; responses by the candidate employee detailing any specific safety or health conditions that could negatively impact the performance of assigned duties, and the results of medical qualification examination performed by the occupational medicine practitioner to assess whether the candidate employee can cope with the job duties, functional requirements and environmental factors of the assigned position.
